Clinical Trials data to Standard disease categories

Hi All,

I have collected Clinical trial information for all drugs with their respective diseases. Now I want to sort all the drugs based on the diseases categories for which I need my diseases name to be uniform. The diseases names in clinical trial data is not uniform as shown bellow as an example:

Clinical_ID Diseases
NCT02256592 HIV
NCT00385554 HIV Infections
NCT00408538 HIV Infections
NCT01575405 HIV Prevention
NCT01694407 HIV
NCT02235662 HIV Contraception
NCT02245945 HIV Prevention
NCT02256592 HIV
NCT02659306 HIV Infection

Now is there any way to make all the disease name map with the standard disease name? like

    Clinical_ID Diseases
    NCT02256592 HIV
    NCT00385554 HIV
    NCT00408538 HIV
    NCT01575405 HIV
    NCT01694407 HIV
    NCT02235662 HIV
    NCT02245945 HIV

Similarly there are many such cases where the disease names are different though they are from same category. Is there any way where we can directly map the standard disease names with clinical trial data?

or any suggestion?!
