Meaning of abbreviations of last-follow-up status in the 2000 breast cancer dataset?

I'm currently working on the 2000 breast cancer dataset: http://www.nature.com/nature/journal/v486/n7403/full/nature10983.html

There is a last_follow_up_status column in the phenotype data. The four levels of this factor are:

a d d-d.s. d-o.c. null

Since I cannot find out any description of this column in their supplementary information, I have to make a guess.

I think 'a' should be alive and 'd' should be dead. But what does 'd-d.s.' and 'd-o.c.' mean? Can anyone help me make a guess?
